A woman has died giving birth to her daughter at University Hospital Galway, the hospital has confirmed.
Teresa Charlin-Foy, 38, died suddenly during the birth on Friday, leaving behind her husband Stephen Foy and newborn baby Claudia Teresa, her funeral notices states.
Her funeral takes place in Renmore, Galway on Tuesday before she is taken to her native home in Sanxenxo, northern Spain.
Mr Foy has been left “devastated and heartbroken” the notice said.
A spokeswoman for University Hospital Galway confirmed a maternal death took place at the hospital last week.
“We wish to express our deepest condolences to the family,” she said.
“While we cannot comment on individual cases, a maternal death is a notifiable incident under the Patient Safety (Notifiable Incidents and Open Disclosure) Act 2023, and, as is the case with all maternal deaths, a full systems analysis review will be undertaken in line with the HSE Incident Management Framework.”
Condolences have been left online from many people including work colleagues of Mr Foy’s at Atlantic Technological University (ATU) Galway and of Ms Carlin-Foy at the Galway offices of ARM and other places she had worked.
One of her colleagues said: “During the time I worked with Teresa, I will always remember her warm smile and the joy she brought to everyone around her.
One man said: “A tragic loss of a wonderful girl. A good friend who was always great fun when we bumped into each other.”
Many people shared their feelings of shock at her sudden passing.
One woman said: “I just wanted to let you know that Tere’s warmth and happiness touched so many lives, that I feel her memory will live on in the hearts of all of us who were lucky enough to know her.”
Ms Carlin-Foy’s profile for work on the Linkedin platform indicates she has worked in the West of Ireland since 2013. Her most recent role was as senior machine learning engineer with ARM.
Earlier this year, three women died in childbirth within days of each other in Ireland including one tragedy at Cork University Maternity Hospital and one at University Hospital Kerry.
Legislation now provides for mandatory inquests as well as the hospital reviews in cases of maternal deaths.
